The Tax Department staff are available to prepare and file municipal income tax returns for residents with fewer than five (5) W-2 forms through April 1, 2026.
After April 1, 2026, all tax documents—including W-2 forms, tax returns, and supporting schedules—must be dropped off at the Customer Service Window. These documents will be stamped “Received,” and a receipt will be provided to confirm the return was filed on time through April 15, 2026.
Please note: Tax Department staff will NOT prepare tax returns at the window after April 1, 2026. All submissions will be drop-off only.

Penalty – Unpaid Tax
Late filing Penalty- as of 1/1/2016
A late filing penalty may be imposed at the rate of $25 per month (or fraction of a month) that a return, other than an estimated income tax return, remains unfiled. This late filing penalty applies regardless of the liability on the return. The late filing penalty shall not exceed $150 for each failure to timely file.
Late filing Penalty- as of 1/1/2024
A late filing penalty will be imposed at the rate of $25 if the return is not filed by April 15, 2024. Only valid extensions will be honored, and relieve the imposed penalty. This late filing penalty applies regardless of the liability on the return.

A late filing penalty will be imposed at the rate of $25 if the return is not filed by April 15, 2025. Only valid extensions will be honored. This late filing penalty applies regardless of the liability on the return.
A penalty may be imposed on unpaid income tax, including unpaid estimated tax, equal to 15% of the amount not timely paid.
A penalty may be imposed on unpaid employer withholding tax equal to 50% of the amount not timely paid.
Interest – Unpaid tax
Interest shall be imposed on all unpaid income tax, unpaid estimated income tax and unpaid employer withholding tax. The interest rate to be applied each calendar year is the federal short-term rate, rounded to the nearest percent. plus five percent (5%). The rate shall apply for the calendar year next following the July of the year in which the federal short-term rate is determined under Section 1274 of the Internal Revenue Code.
The Interest Rate for calendar year 2025 is 9.00%.
The Interest Rate for calendar year 2024 is 10.00%.
The Interest Rate for calendar year 2023 is 7.00%.
The interest rate for calendar year 2022 is 5.00%.
The interest rate for calendar year 2021 is 5.00%
The interest rate for calendar year 2020 is 7.00%
The interest rate for calendar year 2019 is 7.00%
Tax filings and tax payments related to tax years beginning before January 1, 2016 are subject to the penalty and interest rates and late filing penalties. if any. imposed by each municipal income ta,'( ordinance in effect prior to January 1. 2016 – regardless of the date of filing or payment. Ohio Revised Code Section 718.27.

Employers with 10 or more employees are required to file their annual reconciliation and submit all electronic W-2 and 1099-NEC forms through Tax Connect on the City of Brook Park website.
CDs and USB drives are no longer accepted. Any submitted physical media will be destroyed upon receipt.
